
At Children’s Hope Alliance, we are dedicated to supporting young people as they build bright futures for themselves. As part of this commitment, we are excited to introduce Healthy Transitions, a new program designed to support transition-aged youth (ages 16-25) as they navigate the transition into adulthood.
What is Healthy Transitions?
Aging into adulthood can be an overwhelming experience, especially for young people who are facing mental health challenges, housing instability, and/or difficulty accessing resources. Healthy Transitions is a youth-driven program that focuses on bridging these gaps, ensuring that youth and young adults have the support they need to build stable, independent lives.
Through this program, we provide:
Peer Support & Mentorship – Connecting youth with a Youth Peer Support Partner, and families with a Family Peer Support Partner, who have their own lived experience, to foster hope and resiliency.
Education & Employment Assistance – Supporting young people in finding job training, career opportunities, and educational pathways.
Care Coordination – Ensuring youth feel supported and connected as they navigate multiple systems and services.
Youth Voice & Leadership – Creating opportunities for youth to influence the systems and services that impact their lives.
Why Healthy Transitions Matters
Healthy Transitions is more than a program, it’s a movement toward empowering young people to take charge of their futures. Many transition-aged youth (TAY, ages 16-25) fall through the cracks when aging out of youth-focused services and moving into the adult-serving world. Healthy Transitions wants to ensure that no young person is left to navigate this journey alone. Our goal is to make services more accessible, to reduce stigma around mental health challenges, and to empower young people to take charge of their futures, and to have a voice in their community. We want every emerging adult to feel like they have the proper support system and safety net as they figure out what adulthood and independence look like to them. Together, we can create a future where every young person has the support they need to succeed.
